The pharmacist writes why it is still safe, then disburses.
- Ibuprofen after missed fluids in a vaso-occlusive crisis asks for a written reason.
- An antibiotic on a working malaria note is flagged when no bacterial diagnosis is written.
- Artesunate and artemether/lumefantrine on one chart are flagged so the courses do not overlap.
- A second paracetamol asks for the day's total first.
- Pentazocine — vomiting stays visible on the folder.
Adverse effect
Pharmacist, doctor, or nurse files what happened, how severe it was, and whether it is still happening. A severe report is what locks that drug afterwards.
